You will independently prescribe appropriate medications, evaluate,and where necessary, refer patients to other specialists. Your responsibilitieswill include providing care through face-to-face appointments, telephoneconsultations, and home visits.Working within your professional scope of practice andagreed limits, you will offer expert clinical advice to patients, carers, andcolleagues, ensuring that clinical safety and excellence are always maintained.Key responsibilities also include prioritising and triagingpatient needs, making timely referrals for further investigations, andeffectively managing your patient caseload. Core responsibilities include, but are not limited to:Collaborative Care: Work as part of a multi-disciplinary team to deliver safe, high-quality care, supporting the overall health and well-being of a diverse patient population.Patient Assessment and Diagnosis: Serve as the first point of contact for patients presenting with undifferentiated and undiagnosed conditions. Conduct thorough history-taking, physical examinations, and use critical thinking in clinical decision-making to establish a diagnosis and develop a comprehensive management plan.Acute and Chronic Condition Management: Diagnose and manage both acute and chronic conditions, integrating drug- and non-drug-based treatments into individualised care plans, while proactively identifying and managing treatment plans for patients at risk of developing long-term conditions.Holistic Care: Provide holistic, patient-centred care by developing ongoing support and care plans for patients and their families, with a focus on prevention, self-care, and overall health promotion.Health Promotion and Education: Promote health and well-being using health promotion strategies, health education, and therapeutic communication, encouraging patients to adopt healthier lifestyles and practice self-care.Clinical Leadership: Provide leadership within the practice, mentoring and supervising the nursing team, including healthcare assistants, and fostering an environment of evidence-based practice. Contribute to the delivery of key performance indicators, Quality and Outcomes Framework, and other performance monitoring systems.Medication Management: Prescribe and review medications based on the latest evidence-based practices, national and practice protocols, and within the scope of your qualified practice. Offer guidance on prescribed and over-the-counter medications, including advice on side effects, interactions, and regimens.Care Coordination: Coordinate patient care plans, ensuring patients and their families are actively involved in the decision-making process. Work with patients to support adherence to prescribed treatments and interventions.Diagnostic Investigations: Request and interpret diagnostic investigations, such as x-rays, blood tests, and scans, within agreed protocols, to support accurate diagnosis and treatment.Clinical Record Keeping: Maintain accurate and up-to-date patient records in accordance with NMC guidelines and other relevant standards, ensuring patient safety and continuity of care.Triage and Urgent Care: Prioritise and triage patients' health problems, intervening appropriately in complex, urgent, or emergency situations, including initiating effective emergency care when needed.Mental Health Support: Assess, identify, and refer patients presenting with mental health needs in accordance with national guidelines, such as the National Service Framework for Mental Health.Community-Based Care: Provide care both in practice premises and in patients homes, ensuring that all interventions are delivered safely and effectively, with appropriate referrals to other agencies when necessary.Contractual Support: Assist the practice in fulfilling contractual requirements by supporting clinical tasks and contributing to the delivery of high-quality patient care.Evidence-Based Care: Provide care in accordance with NSF, NICE guidelines, and the latest evidence-based practices, ensuring consistency and excellence in clinical delivery.Care ManagementCommunicates confidential and sensitive information to patients, relatives and carers in relation to their condition.Provide highly specialised care advice beyond the scope of normal Nursing practice e.g. care treatment plans, making referrals as appropriate and including fast track referrals.Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking.To monitor and meet care outcomes against standards and recommend or initiate changes as necessary.Health ImprovementManagement of health screening programmes and ensures other team members are aware of health priorities and screening objectives and programmes.Innovation in practice to meet ongoing demands on the practices delivery of care.Improve health outcomes for patients by introducing new developments and can demonstrate them as reports and presentations where appropriate.Supports practitioners in reporting the practice population to promote health improvement and disease prevention.Makes referrals to appropriate agencies -statutory and voluntary where necessary.Administration and professional responsibilitiesProduce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.Participate in the administrative and professional responsibilities of the practice team.Accurate and legible notes of all consultations and treatments are recorded in the patients notes.Ensure clinical systems kept up to date, recording and/or amending accurate details.Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator.Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.Attend and participate in practice meetings as required.Restocking and maintenance of clinical areas and consulting rooms.Training and personal developmentMaintain up to date skills and knowledge, maintaining awareness of professional issues at an advanced level.Training needs will be monitored by yearly appraisal and will be in accordance with practice requirements.
