You will be working with a team of experiencedsupport workers and two clinicians.
This role reports to the Service andRelationship Manager.
You will be partof the Whanganui Regional Leadership Team.
You will also bepart of the Central Region Nurses special interest group where best practice isevolved, portfolio development is supported and professional developmentoccurs.
The role of the Community Mobile team isto help people living independently to flourish, assist with goal planning,mental health and physical health assessments linked to a core belief thatmental health and addictions is our priority, but total wellbeing is our goal.
Each person has different needs, so we tailor the amount and type of support tothe person.
This is a full-time position, workingMonday to Friday 8.30am – 5.00pm with some flexibility.
This position is basedin Whanganui and has an on-call after-hours component which works on arotational basis.
Responsibilities include: Provide consultation, liaison, and development of meaningful and collaborative partnerships across a number of NGOs in the Whanganui Region.
Provide regular coaching and be focused on inspiring every member of your team to exceed their personal best in everything they do.
Focus on physical health and functioning for our tangata whai ora as it affects recovery and wellbeing.
Provision of education and support sessions to kaimahi and tangata whai ora.
Developing contemporary solutions to support exceptional service delivery.
You | Mou Have a current New Zealand registration as a registered nurse.
Full New Zealand's drivers' licence Demonstrated leadership and ability to support and inspire a team.
Demonstrated ability to work well with other services such as Te Whatu Ora, PHOs, and the wider community and social sector.
