About the role: Be part of a dynamic and supportive multi-disciplinary team that effectively uses social work processes to ensure quality health Social Work is provided to Health NZ | Te Whatu Ora – Te Tai Tokerau clients/families/whanau; whilst maintaining professional practice standards.
Our Social Work team works across health services to provide specialist social assessments and planned social work intervention.
The purpose of this position is to provide an effective and responsive Social Work Service to clients in the Kaipara while operating as a member of both inpatient and community multidisciplinary teams.
This role is a full time, permanent 1.0 FTE position, working 40 hours per week in the hospital and the community.
