Details of the offer

Pou Tohu - MH&A Referral Coordination and Service Facilitation Health New Zealand - Te Whatu Ora - Taranaki – New Plymouth, North Island
We are looking for people that share our vision by providing the best care for our people and our community. We have a responsibility to see the whole picture, the whole community, in terms of health and to support everyone working in Taranaki to do the best job they can in a world of constant change.
Taranaki - A place like no other
Taranaki boasts some of New Zealand's most iconic natural landscapes like Mt Taranaki and many adventure activities. The rugged and wild coastline is home to more than 12 world-class surf breaks. Taranaki is a place where you can ski and surf in the same day. For a more relaxing experience, there are many walks (bush, coastal or mountain) as well as art trails, festivals and award-winning museums and galleries and a thriving café culture.
At Health New Zealand | Te Whatu Ora Taranaki we are committed to recognising the value our people bring to our organisation within a supportive team of healthcare professionals that provide comprehensive care to our patients. We believe having a workforce that is diverse and inclusive makes us a pro-equity Health Organisation, better positioned to understand and serve our community.
When you choose to work with us, we will offer you:
A comprehensive orientation and induction to our organisation and your department
Access to clinical training and education
Up to date facilities
Opportunities to participate in local project and regional collaboration teams
The Pou Tohu position provides referral coordination & triage for primary and out of area referrals to our South Adult mental health team. It also provides Service Facilitation/navigation across the team and the wider mental health and addictions sector for whai ora to access community-based support services and residential services.
As the Pou Tohu, you will be instrumental in ensuring whai ora and whanau have a positive first experience engaging with mental health services with an emphasis on whakawhanaungatanga and manaakitanga in all interactions, laying the foundation for positive engagement and support.
Service facilitation and development of a support plan is also vital to enable access to individualised support services based on assessed needs in partnership with whai ora, their whanau and the wider multi-disciplinary team, enabling whai ora to live well in the community.
To be successful in this role, you must have a passion for the mental health field with a priority for improving the quality of lives of people experiencing mental health distress or living with a significant mental illness.
Our ideal candidates will have advanced mental health assessment skills, a broad range of experience in mental health services across service settings, and a good knowledge base of services available in Taranaki both within and focused on specialist mental health providers but also community-based and primary services to signpost whanau to.
