This role will provide lots of variety working as part of an interdisciplinary team.
This exciting opportunity has arisen where we are able to offer a part time flexible hours position for an experienced Pharmacist who will support a new national initiative "Comprehensive Primary Community Teams" (CPCT) in primary healthcare in the Mid North.
This is a different role and model of supporting care in general practice, and we're interested in those Pharmacists that might like some variety in the work that you do. Those who maybe want some flexibility and who have some capacity and interest in supporting and being integral in the success of this initiative. Up to 16 hours per week and may support one or more general practices.
This new initiative aims to support an increase in access to services, and equitable health outcomes through integrating a range of roles including Pharmacists, Physiotherapists, Care Coordinators and Extended Care Paramedics, into existing general practices. This model along with improving health outcomes has a strong focus on expanding the skill mix available to primary healthcare teams and includes support for specialist pathways development for each of the roles. With a priority focus on Maori, Pacific and rural people, these teams are intended to help reach more of our at-risk communities and reduce barriers to accessing care.
About Mahitahi Hauora
Mahitahi Hauora is a primary health entity underpinned by Te Tiriti o Waitangi, our Kaupapa includes ensuring whanau and communities are able to achieve self-determined wellbeing, access to the services they determine they need, and to live a long and healthy life. As the largest PHO in the Tai Tokerau region, we work collaboratively with community, primary healthcare providers and key partners to support general practices, Maori health providers and social sector agencies, in achieving better health outcomes for our Tai Tokerau population.
The Position – Pharmacist
The primary purpose of this role is to provide within scope care directly to people and their whanau, as an integrated member of the general practice interdisciplinary team ensuring accessible, equitable, and optimal use and management of medication treatments.
Working closely with the General Practice interdisciplinary team (IDT), you will be actively involved in meetings and using the systems and processes that support IDT functioning and outcomes. A key purpose of this role is to support increasing the capacity for other members within the general practice interdisciplinary team, with key functions that include:

Medicines review, management and optimisation
Clinical evaluation and management of medication use
Medication reconciliation
Advising and supporting whanau in understanding medication treatments, and impacts on their health and wellbeing
Supporting management of long-term conditions through initiation and continuation of prescribed treatments within area of practice (Prescriber scope only).
Improve awareness and management of medicines-related harms
Improving management of high-risk medicines particularly relating to at-risk populations.

Successful applicants will need to meet the following criteria:

Bachelor of Pharmacy (BPharm) or equivalent overseas qualification
Qualified pharmacist with current APC without restrictions
Pharmacists undertaking prescribing activities must be registered and have a current APC (without restrictions) in the Pharmacist Prescriber Scope of Practice.
Holds, or is working towards, relevant postgraduate clinical pharmacy qualification (preference to Postgraduate Diploma level or equivalent)
Demonstrate cultural competence and understanding of Te Tiriti o Waitangi in action
Comprehensive understanding of the inequities in access and use of medicines in Aotearoa New Zealand and its causes
Advanced written and verbal communication skills
Excellent time management and organisational skills
Excellent critical appraisal skills, being able to identify the best evidence-informed solutions to clinical and practice questions and issues.

While not essential, the following would be an advantage:

Several years of recent experience in working within interdisciplinary clinical care teams – in primary or hospital settings
Experience in leading and advising other health professionals or providers of care
Pharmacist Prescriber Scope of Practice

You must be a NZ Citizen or Permanent resident or have a current open NZ work visa. (Not tied to an accredited employer)
