Salary: £600 - £600 per day, 3 month-contract + extension options
You will be joining a very cohesive and collaborative PCN in the Christchurch area of Dorset who are looking to take advantage of the newly/recently-qualified GP funding – to be applicable for this role therefore you must be within two years of your CCT date and have not had a Salaried role in the time since. You must be qualified as a General Practitioner and registered with the GMC and UK performers list. Working as their Frailty Lead GP, you'll liaise with the Frailty teams in each practice to plan and provide the routine care for the various care homes they PCN looks after collectively (acute patients will still be seen by the practices' On the Day teams).
With 11 care homes in the patch, you won't visit each one every week, so you'll essentially manage your own diary and in line with the Frailty teams and individual practice MDTs, you'll largely design the role yourself.
Given the nature of the work, the hours are very flexible and if doing half days, your 4.5-hour session can cover any period between normal GP hours.
Sessions: 4 x 4.5-hour sessions, ideally all half days but 2 full days or 1 full and 2 half would be fine as well.
