It weaves the functions of the20 former District Health Boards into its regional divisions and districtoffices, ensuring continuity of services in the health system. Mo te tunga | About the role Our Palliative Care team are dedicated to delivering exceptional care andsupport to patients facing life-limiting illnesses. Our multidisciplinary teamis committed to providing holistic, patient-centered care that focuses onrelieving suffering and improving quality of life. We are looking an SMO thatwill provide support to the West Coast, in a 12 months, fixed term, Part Timecapacity (0.3 FTE). Responsibilities:The successful candidate will provide expert palliative care consultation and management to patients withadvanced illness who are resident on the West Coast, working in partnershipwith the specialist palliative care team in Waitata Canterbury. Collaborate with the local palliative care nurses and the wider, multidisciplinaryteam (including primary providers and hospital clinicians) to develop andimplement individualised care plans. Offer support and guidance to patients, families, and caregivers regardingtreatment options, symptom management, and end-of-life care decisions. Assist with developing and running educational initiatives for healthcareprofessionals and community outreach programs to raise both skills and awarenessin palliative care. Contribute to quality improvement projects, research activities to advancepalliative care provision on the West Coast and ensure sustainability ofservice into the future.
