You will develop strong links with primary health practice to support patients and their whanau in the Kapiti community.The role will involve all aspects of patient management from triage to discharge, so the ability to confidently manage patient flow is necessary. The role involves home visits, joint work, and liaison to ensure our Kapiti patients receive timely care.The Kapiti Older Adult, Rehabilitation and Allied Health Community Team is a multidisciplinary team of community-based Nurse practitioners, Liaison Nurses, Geriatricians, and Allied Health clinicians. Our team would like to see another Nurse Practitioner join us as we aim to provide a more timely, flexible, and collaborative service to primary care to help older people remain healthier and living in their own homes. The team works in a person-centred and integrated model, enriching the scope of primary care, responding quickly to the acute frail person, and promoting proactive approaches to optimise the functional trajectory of our older people.The team will work closely with GPs and other community healthcare providers to support healthy ageing in the community to optimize the population health. The team will also have close links with the Health Care Home and inpatient medical services while working alongside local Maori health providers.The person in this role will:Be registered as a nurse practitioner with the nursing council.Have some experience in community or acute health of the older person.Have the ability to co-work alongside other members of a busy allied and medical team.Possess excellent communication and interpersonal skills and the ability to work effectively with all levels of staff in a multidisciplinary team.Have a strong patient care focus and be able to deliver high quality of care for patients and whanau in their homes.Have a commitment to patient-led care, by building collaborative, communicative, and respectful relationships.Show a passion for ongoing learning, professional development, and the ability to share, support, and develop others.Here at Capital, Coast & Hutt Valley District, we embrace diversity and welcome applications from all people, including all gender identities, ages, ethnicity, sexual orientations, disability, and religions.
