The Role The Manaaki Ora Nurse will deliver mobile nursing services to whanau referred to Manaaki Ora, with a focus on heart disease, diabetes, and lung disease.
You will work closely with other healthcare professionals to ensure that services are effectively targeted and accessible to Maori and Pasifika communities, while also serving the entire population.
You will enable individuals to effectively manage their own health with informed decision-making.
The role incorporates the functions of district nurse, public health nurse and practice nurse.
Key Responsibilities Conduct early identification and intervention for heart disease, diabetes, and lung disease among Maori and Pasifika communities.Improve access to primary and secondary healthcare services for whanau, focusing on clinical and cultural competency.Conduct kanohi ki te kanohi (face-to-face) consultations to conduct a cultural assessment and develop Whanau Health Plans to address needs incorporating Taha Tinana, Taha Wairua, Taha Hinengaro, and Taha Whanau.Facilitate Hauora wananga to educate on Maori perspectives on health, in settings such as the home, marae, workCollaborate with stakeholders to maintain productive partnerships and uphold the values of Manaaki Ora and Otakou Health Limited.Manage resources, support strategic planning, and ensure the achievement of organisational objectives and key performance indicators (KPIs).Qualifications & Experience Registered Nurse with a current practicing certificate.Experience working within Maori or Pasifika communities, with an understanding of their health and social needs.Demonstrated ability to provide culturally competent care, respecting Maori values and principles.Strong organisational skills with experience in service coordination and stakeholder collaboration.Ability to work independently in a mobile capacity and manage clinical and administrative tasks effectively.Application To submit your application, click the apply button and include a cover letter and CV Applications close on Friday 1 November and will be reviewed the following week.
