Mo te tunga | About the Role:Join our High and Complex Needs (HCN) Interagency Unit, where we collaborate with multiple government sectors and non-government agencies to develop effective solutions for tamariki and rangatahi facing high and complex needs.As an HCN Specialist, you will lead the coordination and facilitation of the Interagency Team (IAT), ensuring that we provide tailored support plans that build skills and promote a positive future for tamariki and their families.
This role balances people engagement and administrative responsibilities, requiring sound clinical knowledge, strong self-motivation, highly developed organisational skills, and communication skills.Key Responsibilities:• Collaborate closely with professionals, whanau and tamariki to create intensive support plans.• Coordinate services and interventions to foster positive changes.• Produce high-quality documentation and reports for stakeholders.• Build respectful working relationships across various agencies.
