You will coordinate care with other healthcare providers and community services, ensuring a holistic approach to patient rehabilitation and recovery. In line with Te Tiriti o Waitangi, you'll provide culturally respectful and equitable care, especially supporting Maori health outcomes and acknowledging diverse cultural values, including age, gender, ethnicity, and sexual orientation. Your role includes administrative tasks such as accurate clinical documentation, promoting Primary Health Organization enrollment, and adhering to health, safety, and privacy regulations.
