The Bay of Plenty Care Coordination Centre serves as a centralised access point for patients needing short-term health services across the region. We are looking for an experienced Registered Health Professional to join us as a Community Care Coordinator based at Te Whare Whakamana building located on 17th Avenue, across from the Historic Village.
The position involves compassionate client interactions and collaborative work with healthcare professionals, using phone and technology-based communications to guide clients toward the best available services.
This is a permanent, full-time position at 1.0 FTE (40 hours per week) and requires flexibility to cover rotating weekend shifts.
Success in this role requires a solution-focused mindset, a collaborative approach, and an eye for identifying opportunities to enhance service integration. This is a dynamic, agile environment where no two days are the same, offering both challenges and growth as the Coordination Centre continues to develop.
Responsibilities include screening requests, determining appropriate responses, and fostering key relationships with healthcare providers across the community and hospital settings.
