We have an exciting new role available to further expand the amazing Hawkes Bay clinical pharmacist facilitator team. We have a vacancy available for a Clinical Pharmacist based as part of the Te Whatu Ora Te Matau a Maui, Hawkes Bay Pacific health team in Hawkes Bay. This role will be fully integrated within the Pacific health team and support transition of care and appropriate prescribing of medicines in Pasifika patients and their fanau. The Hawkes Bay clinical pharmacist facilitator team is the largest within New Zealand and has a great support structure with its own team manager, two team leaders and a clinical educator. This close-knit autonomous team of Clinical Pharmacists offers a supportive environment in a role that will allow you to reach your potential to work at top of scope. Your main responsibility is to provide direct clinical pharmacist facilitation services, focusing on the appropriate prescribing and utilisation of medicines to the Hawkes Bay Pasifika community. To apply for the role, you must have the following: Registered as a Pharmacist with the Pharmacy Council of New Zealand holding a valid Annual Practising Certificate (APC) with no restrictions. Identify as Pasifika and be able to communicate with at least basic Pacific language skills. Work experience sufficient to demonstrate general pharmaceutical knowledge in core areas and pharmaceutical knowledge in management of long-term conditions. Current Driver Licence. Great communication and interpersonal skills Motivation to improve services and reduce inequities for Pasifika patients and their fanau Desire for lifelong learning and development as a pharmacist Research, interpretation and provision of medicine information.
